I have used the Emirates Skywards contact form to submit my request. In the form, I have picked Emirates Skywards as the topic and Membership Card as the subtopic. I have also attached my QR membership card as well as my statement for the last 6 months. Since I have an upcoming 4-sector return flight in J class, I have included the booking confirmation in the request as a mean to show that I am serious about switching my loyalty to them. Initially, I have requested a status match to Gold but I was told that they can only do Silver and asked if I still want to proceed with this. Two days later, I check my Skywards account to find out that I was bumped from Blue to Silver.
