Submitted match request along with photo of Companion Pass and driver's license. Had multiple flights booked/flown with Alaska same year, including flights to Kauai, and 1st class transcontinental. Alaska only matches Southwest's Companion Pass if you also provide proof of A-List/Preferred status.
