I went to the contact page. It's weird, because it continually asks me to enter my date of birth. Anyway, after a few mistakes, I finally got it right and got the following email, I quote:
We appreciate that you're looking to fly better with us. I'd be happy to assist you with your status match request.
I’ll need any of the following documents.
Copy of six months (Jul 2019- Jan 2020) activity statement with Delta showing your Tier status
Copy of your Delta frequent flyer card (digital or hard copy) showing the validity
Please log in through the below link and attach your document.
Once I receive them, I'll review your request and get back to you with the next steps.

So I was able to get a pdf of my Medallions statement from July 1st to January and I attached that. I didn't know about the card since with the current technology I don't have a physical card and I don't see an electronic image of a card so I took a screen grab of my My SkyMiles on the delta.com
page (but not on the app, just the actual .com page) and I attached that.
Then I got my second response, I quote:
Thank you for your query – we will respond as quickly as possible.
