Contacted via online form. Communication continued via email. Initially requested status match with explanation of why I was "match-worthy". Was then requested to send in a filled in status match form (they sent a template), a prior account statement, an upcoming booking with them and pictures of my membership card with the other program. Also got asked for some basic details about my Singapore Airlines/KrisFlyer account.
