Reached out via online chat on 27/Sep, received email from kf_contact within 2 hours including request form which also asking for FFP card copy and 12 months flights activity. Received further email on 8/Oct that request was approved, and confirmed via KF app login. I do have 2 existing bookings with SQ at the time of request (1 eco + 1 biz) which are shared in the form, and another 1 biz booking right before the request approval.
