I completed the form on statusmatch.com on 10/13. Required a screenshot of CP and a copy of my account details. Southwest only shows the first name and they needed both first and last. Matched it to 50k after resending my screenshot. Technically this is a challenge since you have to fly one Air Canada flight (I don't recall if it's a return flight) and then status is matched for another 12 months.
